Every advanced procedure on Fitzpatrick IV–VI begins weeks before the device touches the skin. Pre-conditioning is non-negotiable.

4–6 Weeks Prior
Discontinue retinoids, hydroquinone, and exfoliating acids based on tolerance. Begin a barrier-supportive routine: gentle cleanser, niacinamide, ceramides, and broad-spectrum SPF 50 daily.

2–4 Weeks Prior
Initiate melanocyte-suppressive priming when clinically indicated (tyrosinase inhibitors, azelaic acid, tranexamic acid). Avoid sun exposure, self-tanners, and waxing in the treatment zone.

7 Days Prior
Stop actives that compromise the barrier. Confirm no active infection, herpetic outbreak, recent isotretinoin, or compromised skin in the treatment area. Initiate antiviral prophylaxis for perioral procedures.

Day Of
Arrive clean-skin, no makeup, no SPF. Reassess Fitzpatrick, photo-document, and confirm device parameters calibrated for IV–VI: lower fluence, longer pulse durations, extended cooling, conservative passes.

First 24–72 Hours
Cold compresses, occlusive barrier repair, and bland emollients only. No actives, no heat, no exercise, no sun. Sleep elevated. Mineral SPF 50 reapplied every two hours when outdoors.

Day 3–7
Reintroduce niacinamide, peptides, and growth factors as the barrier closes. Continue tyrosinase inhibitors to suppress melanogenesis. Monitor for early signs of PIH — intervene immediately if erythema lingers.

Week 2–4
Reintroduce actives slowly: azelaic acid, low-dose retinaldehyde, vitamin C. Schedule pigment-support adjuncts (oral tranexamic acid, polypodium, glutathione) when appropriate. SPF remains non-negotiable.

Long-Term Maintenance
Maintenance pigment suppression, quarterly photo-documentation, and an annual barrier audit. Treatment cadence is spaced longer than standard protocols to honor melanocyte recovery and prevent rebound pigmentation.
